Qualifications
- Proof of Training or successful completion of Home Health Aide or STNA training program and or certification, if required by law or agency policy.
- High school diploma or GED preferred.
- Experience in personal care, nursing home, hospital, or home health setting is preferred not required.
- Good physical and mental condition sufficient to perform job duties.
- Strong interpersonal, communication, and observation skills.
- Ability to follow instructions accurately and work independently within assigned scope of practice.

Registered Nurse

The Registered Nurse is responsible for providing skilled nursing services, conducting patient assessments, developing and revising the plan of care, coordinating with physicians and interdisciplinary team members, and supervising home health aide services as assigned. This position requires strong clinical judgment, communication skills, and the ability to work independently in the home setting.
Qualifications
- Current license as a Registered Nurse in the state of practice. Associate degree in Nursing required; Certificate in Nursing preferred.
- Minimum of one year of nursing experience preferred not required; home health, acute care, or public health.
- Ability to make independent nursing judgments in the home setting.
- Strong interpersonal, organizational, and communication skills.
- Ability to work with patients, families, physicians, and other healthcare professionals.
- Demonstrated integrity, initiative, sound judgment, and analytical ability.

Licensed Practical Nurse

Full-time / Part-time
The Licensed Practical Nurse (LPN) is responsible for delivering skilled nursing services to home health clients under the supervision of a Registered Nurse. The LPN performs clinical assessments, administers medications, provides wound care, and maintains accurate visit documentation in full compliance with Ohio Medicaid standards and RHS policy. This is a field-based role serving clients across the Toledo metropolitan area.
Minimum Qualifications
The following are required at the time of hire and must remain current throughout employment:
- Active, unencumbered Ohio LPN licence in good standing
- Current BLS / CPR certification (AHA Healthcare Provider)
- Valid driver's licence and reliable personal transportation
- Current I-9 employment eligibility documentation
- Ability to lift and transfer clients safely (up to 50 lbs with assistance)
- Proficiency with a smartphone for EVV and clinical documentation
- Strong written documentation skills — Medicaid-standard accuracy required
- Ability to work independently in a field-based environment
Preferred Qualifications
- 1+ year of home health or long-term care nursing experience is preferred, not a requirement. ● Experience with Electronic Visit Verification (EVV) systems
- Wound care or IV therapy certification or additional training
- Familiarity with Ohio Medicaid billing documentation standards
- Experience with Alora or similar home health software
- Bilingual — Spanish, French, or other languages a plus
