JOB DESCRIPTION: Registered Nurse, Licensed Practical Nurse

Status: Non-Exempt

Reports To: Clinical Supervisor / Director of Nursing

Supervises: Home Health Aides and other assigned staff

Position Summary

The Registered Nurse is responsible for providing skilled nursing services, conducting patient assessments, developing and revising the plan of care, coordinating with physicians and interdisciplinary team members, and supervising home health aide services as assigned. This position requires strong clinical judgment, communication skills, and the ability to work independently in the home setting.

Essential Duties and Responsibilities.

  • Perform initial and ongoing nursing assessments of the patient’s physical, psychosocial, and emotional needs in accordance with agency criteria and applicable regulations.
  • Observe, assess, and report significant clinical findings or changes in patient status to the appropriate physician and/or supervisor.
  • Provide skilled nursing services within the RN scope of practice and agency policies.
  • Develop, implement, and revise the nursing plan of care in collaboration with the physician, patient, family, and interdisciplinary team.
  • Coordinate care with other team members, including therapists, case managers, and support staff.
  • Supervise and support Home Health Aides as assigned.
  • Educate patients, caregivers, and family members regarding care plans, medications, safety, and disease management as appropriate.
  • Document all patient-related communications, visits, assessments, interventions, and outcomes accurately and in a timely manner.
  • Ensure documentation supports reimbursement requirements and compliance with agency standards.
  • Uphold infection control standards, universal precautions, and all applicable safety procedures.
  • Respond appropriately to emergencies and follow agency emergency procedures.
  • Maintain confidentiality, patient rights, and professional standards at all times.
  • Demonstrate timely responsiveness and excellent customer service in all interactions.
  • Perform other related duties as assigned.


Qualifications

  • Current license as a Registered Nurse in the state of practice.
  • Associate degree in Nursing required; Bachelor of Science in Nursing preferred.
  • Minimum of one year of nursing experience required; home health, acute care, or public health experience preferred.
  • Current TB test and health screening as required by agency policy.
  • Ability to make independent nursing judgments in the home setting.
  • Strong interpersonal, organizational, and communication skills.
  • Ability to work with patients, families, physicians, and other healthcare professionals.
  • Demonstrated integrity, initiative, sound judgment, and analytical ability.


Physical and Environmental Demands

  • Must be able to hear, see, speak, sit, stand, bend, stoop, climb stairs, and travel to patient homes.
  • Must be able to work in a variety of home and community settings.
  • Must be able to drive in various weather conditions.
  • Must be able to perform duties in accordance with physical requirements of home health care.
